How to download a birth or death certificate in Madhya Pradesh, what to do when the record is wrong or missing, and when a refusal becomes a legal matter.

How-To Guide · Madhya Pradesh

You need a birth or death certificate in Madhya Pradesh for a school admission, passport, insurance claim or property transfer, and the portal shows no record, or the record has a wrong name or date. To download a certificate in Madhya Pradesh, you search the registered record on the state or national civil registration portal by registration number, or by name and date, and download the digitally signed PDF; if the record is wrong or was never registered, you need a correction or late registration, and a refusal can end up before a magistrate. This guide was last verified in October 2026.

The short answer
  • Download: search by registration number, or name and date, on the national CRS or Madhya Pradesh online portals, then save the digitally signed PDF.
  • Wrong record: file an affidavit and a correction application with the local registrar.
  • Never registered: apply for late registration. Beyond one year, a judicial magistrate order is needed.
  • If refused: it is a legal matter. Keep every document and track the order.

01Why certificates go wrong in Madhya Pradesh

A birth or death certificate is the base document for almost every other record. Schools, banks, insurers, passport offices and courts all ask for it. When it is missing or wrong, everything behind it stops.

The record may not be online yet

Older events were often registered on paper with the local body, such as a municipal corporation, nagar palika or gram panchayat. If that entry was never digitised, a search on the portal returns nothing, even though the event was registered.

The details may be wrong

Spelling errors in the name, a wrong date of birth, a missing parent name or a wrong sex entry are common. A small error can block a passport or a pension claim.

The event may never have been registered

Many births and deaths, especially in villages and older years, were never registered. In that case you cannot download anything. You have to ask for late registration, and the rules get stricter as time passes.

A download works only if the event was registered correctly. If it was not, the fix is a correction or a late registration, not a better search.

02Who can apply and what you need

Anyone can generally ask for a copy of a registered birth or death entry, but the person named, a parent, a legal heir or an authorised person is the usual applicant. Registration is governed by the Registration of Births and Deaths Act, 1969, and the Madhya Pradesh rules made under it.

  • For a birth certificate: child name (if entered), date of birth, place of birth, and parent names. A registration number makes the search quick.
  • For a death certificate: name of the deceased, date of death, place of death, and the registration number if you have it.
  • For your own ID: keep an Aadhaar number or mobile number ready, as the portal may ask for OTP login or ID proof.

The old hospital slip, the registration receipt or an earlier certificate is very helpful because it carries the registration number and date.

03How to download a birth or death certificate online (steps)

The steps below follow the usual online route. Portal screens change, so follow the labels on the live site.

  1. Open the right portal. Use the national civil registration site, crsorgi.gov.in, or the Madhya Pradesh services portals such as mponline.gov.in and mpedistrict.gov.in. Your city or local body may also run its own page for these records.
  2. Choose the service. Select birth or death certificate search, download or print.
  3. Select Madhya Pradesh and the district or local body. The record sits with the local registrar where the event took place.
  4. Search. Enter the registration number, or the name and date of the event. Try spelling variants if nothing comes up.
  5. Verify the entry. Check every field: name, date, place, parents, sex. If anything is wrong, stop and read the correction section below.
  6. Pay the fee if asked. Many online downloads are free or carry a small copy fee. Check the fee shown on screen before you pay.
  7. Download the PDF. A properly issued certificate is a digitally signed PDF. Save it and print it on plain paper if an office asks for a hard copy.

If you do not want to use the portal yourself, an MPOnline kiosk or a Lok Seva Kendra can usually help for a small service charge.

Fees are indicative and set by the state. Always trust the amount shown on the official portal or told by the registrar.

05Correcting a wrong name or date

A correction is a request to the registrar to fix an entry that was recorded wrongly. It is not the same as a fresh registration.

The usual route

  1. Get a copy of the existing certificate or the entry.
  2. Write a sworn affidavit that states the correct details and why the entry is wrong. Our guide to the name change affidavit format shows how an affidavit is usually drafted.
  3. Attach proof of the correct details, such as a school record, hospital record, parent ID or an earlier official document.
  4. File the application with the registrar of the local body that holds the record, and pay the fee.

Under the 1969 Act, the registrar can correct clerical or formal errors in the register. A change that looks like a substantive change of identity may need more proof, and the registrar can refuse.

If the correction is rejected

A rejection turns an office task into a real legal dispute. You may need to appeal within the department, or approach a court. Keep the written refusal, the date and the registrar name, because they will be needed later.

06Late or delayed registration

The Act sets a time limit to report a birth or death, with a short window for normal registration. After that window, the event counts as delayed.

  • Within the normal window: register with the local registrar, with no late fee or a small one.
  • After the window, up to one year: registration needs the written permission of a designated authority, with an affidavit and a late fee.
  • After one year: registration needs an order from a first class judicial magistrate under section 13(3) of the Act.

Courts have dealt with this point. The Bombay High Court has held that a Rule 9 of the Madhya Pradesh rules, which let an executive magistrate act beyond one year, could not stand against section 13(3), so the one-year line goes to a judicial magistrate. A Registration of Births and Deaths (Amendment) Bill, 2026 has also been reported to tighten the rules for long-delayed registrations. Check the current text before you apply.

What to bring

An affidavit, proof of the event such as a hospital record, school record or a village record, a proof of non-availability of the entry from the registrar, and ID of the applicant. For a death, a cremation or burial record or a witness statement also helps.

07When a refusal becomes a legal dispute

Most people finish this process without a court. Trouble starts when the registrar refuses a correction, rejects a late registration, or no one will give a clear written reason.

At that stage you are dealing with a legal matter, with dates, parties and orders. You may need to file an application before a magistrate, or a writ before the High Court at Jabalpur, Indore or Gwalior bench if an authority acts without proper reason. Whatever you file, keep copies of every application, receipt and order, and note each next hearing date.
